Annual Bucket Truck Inspection Report | 3-Part Form

ABTIR-001-10

Keep a paper record of an annual bucket truck inspection with this three-copy carbonless inspection report form. Each legal-size 8.5 × 14-inch form produces white, yellow, and pink copies to distribute the completed report according to your organization’s procedures.

What the report covers

The printed form includes spaces for the inspection date, motor carrier and vehicle details, unit number, aerial-device identification, and inspection results. Its check areas cover:

  • Vehicle and road equipment: tires, wheels and rims, suspension, brakes, lighting, steering, frame, fuel and exhaust systems, windshield components, and other listed items.
  • Aerial equipment: lift chassis, hydraulics, outriggers, lower and upper boom, elbow, platform (bucket), winch, and listed placards.
  • Results and sign-off: OK and repair-date columns, remarks, inspection company details, and the printed name and signature of the qualified inspector.

Check that this form fits your inspection program. Review the sample against your vehicle and aerial device, the manufacturer’s instructions, and applicable federal and state requirements before ordering. Not every printed item applies to every truck. This form documents findings; it does not by itself certify a passed inspection or establish compliance.

Using the inspection report

Record the inspector’s findings, repair dates, and remarks on the form, then distribute the completed copies according to your organization’s procedures.

Not by itself. The form has OK and repair date columns, a remarks area, and spaces for the qualified inspector’s printed name and signature. It does not contain a universal “passed inspection” certification or determine whether noted repairs have been completed. The findings, required corrective work, and applicable inspection procedures must be evaluated separately.

This is an 8.5 × 14-inch paper report, not a decal to mount on the truck. It provides white, yellow, and pink carbonless copies so the completed record can be distributed under your organization’s procedures. Decide which copy stays with the vehicle, inspector, or office according to your applicable recordkeeping requirements; the form itself does not assign those copies.

The form provides a written record with vehicle and aerial-device identification, inspection date, equipment check areas, repair dates, remarks, and inspector sign-off. Whether it meets a particular OSHA, DOT, state, or employer documentation requirement depends on which rules apply to the equipment and whether the completed report contains every required detail.

It organizes checks for both the vehicle and aerial equipment. Printed sections include tires, brakes, steering, lighting and frame items, along with the lift chassis, hydraulics, outriggers, booms, elbow, bucket, winch, and placards. The inspector can mark listed items OK, record repair dates, add remarks, and sign. It is a documentation aid, not a substitute for the manufacturer’s inspection procedure; some listed items may not apply to every truck.

The form provides spaces for the qualified inspector’s printed name and signature, plus the inspection garage or company name and telephone number. Your organization should designate a person qualified under the requirements applicable to that inspection and set its own process for reviewing findings, addressing repairs, distributing the three copies, and retaining records.
