Annual Overhead Hoist Inspection Checklist – 2-Page, 3-Ply Report

AOHIR-001

Record overhead hoist inspection findings on a two-page printed annual inspection report. The checklist provides fields for component condition, comments, test results, deficiencies and the inspector’s signature.

What each complete report includes

Each complete report contains two separate three-ply sets: one set for page 1 and another set for page 2. Each page set includes one white, one yellow and one pink copy. These are separate pages, not a single double-sided three-ply set.

  • Listed page size: 8½ × 12 inches.
  • Choose quantities of 1, 10, 25, 50, 100, 250 or 500 complete reports.
  • Each complete report has six sheets in total: three copies of page 1 and three copies of page 2.
  • This is a physical printed checklist, not a downloadable PDF, editable template or online inspection service.

Page 1: hoist and trolley checks

The form is headed “Over-Head Hoist Annual Inspection Report” and includes company contact details, a date field and a unit-number field. Component columns are labeled N/A, OK, Faults and Comments.

Main-hoist and auxiliary-hoist sections include controls, push buttons, sound, movement, brakes, upper and lower limits, upper and lower blocks, sheaves, rope and connections, drum spooling and hooks. The trolley section includes controls, travel, sound, brakes and trolley/bridge obstruction. Housekeeping, ladders and landings, and oil-leak entries also appear.

Page 2: bridge, tests and findings

The second page continues the checklist with brake operation, loose parts, keepers, lubrication, fire-extinguisher locations, retaining latches, hook movement and hook deformation. The bridge section covers controls, travel, sound, brakes, limits, alarms, lights and rigging capacity.

A Proof Load Test section provides fields for maximum rated capacity, test percentage and test weight, with a Pass/Fail choice. A No Load Operation section has a Satisfactory/Unsatisfactory choice. Separate areas capture comments, deficiencies, inspector signature and date.

Using and keeping the report

Complete both pages to document the inspection actually performed, and keep matching copies of pages 1 and 2 together when distributing records. Follow the equipment manufacturer’s procedures and your inspection program to determine applicable checks, intervals and any testing.

The presence of test fields does not mean a load test is required or appropriate at every inspection. This form does not provide test instructions, qualify an inspector, certify equipment or guarantee compliance. Review both example pages before ordering to confirm that the form suits your equipment and recordkeeping needs.
