Annual Hi-Rail Safety Inspection Checklist – 3-Part Report Forms

AHRSIC-001-10Qty

Document hi-rail vehicle inspection findings with a printed annual Hi-Rail safety inspection checklist. The form provides vehicle-identification fields, checks for installed hi-rail gear, comments and an inspector signature area. Each report has three copies for distributing the completed record.

Size, copies and quantities

  • Listed form size: 8½ × 12 inches.
  • Three-ply format with one white, one yellow and one pink copy per report.
  • Choose packs of 10, 25, 50, 100, 250 or 500 report sets.
  • This is a physical printed form, not a downloadable PDF, online fillable template or inspection service.

Vehicle and hi-rail gear details

The form is headed “Annual Hi-Rail Safety Inspection Checklist” with “FRA § 214.523” below it. Identification fields include owner, date, vehicle make, model, year, VIN, license tag, mileage and the make and model of the front and rear hi-rail gear.

Checklist and findings

The pictured checklist includes:

  • Availability of the manufacturer’s installation and/or maintenance manual, as applicable.
  • Hi-rail assemblies checked for loose or missing parts.
  • Guide-wheel wear compared with the manufacturer’s limits.
  • Tram or alignment compared with the manufacturer’s limits and procedures.
  • Guide-wheel gage compared with the manufacturer’s limits, with front- and rear-axle gage fields.

Sat and Unsat columns provide space to record findings. A section headed “REQUIRED ON NEW HI-RAIL VEHICLES” contains printed regulatory references and checkboxes for a backup alarm and a warning light or beacon. Comments, signature and date fields complete the report.

Scope and recordkeeping

This is a hi-rail vehicle and gear inspection form, not a railroad track-condition checklist, railcar inspection report or general vehicle roadworthiness form. Review the full example image and compare the fields with your equipment manufacturer’s procedures and inspection program before ordering.

The regulatory references and inspection-interval notes shown on the form are printed content, not a determination that the form covers every requirement for your vehicle. A blank form does not perform an inspection, qualify an inspector or certify compliance. Use current applicable requirements and manufacturer procedures to determine the inspection scope, timing and records needed.

The three-copy format lets you distribute copies among vehicle, inspector and owner records according to your recordkeeping procedures.

Separate Hi-Rail inspection stickers
