This machine inspection required warning sticker tells operators that the machine must be inspected and operating properly before use. I-040 measures 3¾ inches wide × 7½ inches tall, with an orange WARNING header, safety-alert symbol, and black instructions on white.

THIS MACHINE MUST NOT BE USED UNTIL IT IS INSPECTED AND OPERATING PROPERLY!

• Do not operate this machine unless you have been properly trained as described in the manufacturers operation and safety manual by a qualified person and authorized to operate this machine. Your training includes reading and understanding the safety, operating and maintenance instructions in the manufacturer’s manuals, knowing your employers work rules and applicable governmental regulations.
• Follow the instructions in the operating manual, the manual of responsibilities and applicable standards for required inspections.
• Do not replace items (I.e. Batteries, wheels, counterweight, etc.) with items of different weight or specification because this will affect stability of machine.
• Do not modify or change machine without written approval from the manufacturer.
• Operate this machine with extreme caution STOP all operations if malfunction occurs.

Improper use of this machine could cause death or serious injury.

This decal communicates inspection and operating precautions. It has no fields for recording inspection dates, inspector names, or results, and does not certify that a machine has passed an inspection. Applying it does not replace required inspections, operator training, equipment manuals, or maintenance records.

When replacing an equipment label, compare the full printed message and 3¾ × 7½-inch dimensions with the original marking and available space. Follow the equipment manufacturer's label-location guidance rather than assuming one cab or access-point location fits every machine. Keep controls, identification plates, and other instructions unobstructed.
