- 93 numbered checks covering general condition, cab, chassis, outriggers, operator controls, load chart, safety devices, boom, tower, load block, and hook.
- Status choices for satisfactory, deficiency, recommendation, and not applicable.
- Load-test results, deficiency and recommendation notes, correction dates, remarks, and examiner name and signature.
